چکیده
Background: Patients with a diagnosis of Spondyloarthritis (SpA) and Psoriatic Arthritis (PsA) may have predominant axial or peripheral symptoms, the frequency distribution these symptoms determine clinical by rheumatologist (“clinical clusters”). Clustering analysis represents an unsupervised exploratory which tries to identify homogeneous groups cases (“statistical clusters”) without prior information about membership for any cases. Objectives: To “statistical clusters” involvement according specific location in whole spectrum SpA PsA (without patients), evaluate whether are agreement “clinical clusters”. Methods: Cross-sectional multicentre study 24 participating countries. Consecutive patients considered their treating as suffering from either PsA, (axSpA) (pSpA) were enrolled. Four different cluster analyses conducted: first one using all musculoskeletal manifestations (i.e., arthritis, enthesitis dactylitis), thereafter each manifestation individually. Multiple correspondence k-means clustering methods used. Distribution characteristics compared across clusters. Results: 4465 included analysis. Two clusters found regard (Fig. 1). Cluster 1 showed low prevalence comparison 2; however, when appeared 1, this was mostly represented arthritis hip, knee ankle, well heel. higher males (63% vs 44%), HLA-B27 positivity (69% 38%) (80% 52%), more frequent axSpA (66% 21%) frequently fulfilling ASAS criteria vs. 41%). 2 psoriasis 25%), (61% 19%), they fulfilled (26% 11%) CASPAR (57% 19%). Figure 1. Three arthritis. Clusters 3 high joint disease, although located predominantly lower limbs 2, upper 3. males, positivity, involvement, presence psoriasis, 3, respectively. dactylitis fulfillment criteria. Information exhibited three groups: very enthesitis, while enthesis Finally, also that dactylitis, toes fingers Conclusion: These results suggest heterogeneous patterns clearly defined groups, confirming clear overlap underlying diagnoses. Treating axial spondyloarthritis and peripheral spondyloarthritis, especially psoriatic arthritis, to target: 2017 update of recommendations by an international task force
Therapeutic targets have been defined for axial and peripheral spondyloarthritis (SpA) in 2012, but the evidence for these recommendations was only of indirect nature. These recommendations were re-evaluated in light of new insights. Based on the results of a systematic literature review and expert opinion, a task force of rheumatologists, dermatologists, patients and a health professional deve...
متن کاملTreating axial and peripheral spondyloarthritis, including psoriatic arthritis, to target: results of a systematic literature search to support an international treat-to-target recommendation in spondyloarthritis
BACKGROUND Current recommendations for the management of axial spondyloarthritis (SpA) and psoriatic arthritis are to monitor disease activity and adjust therapy accordingly. However, treatment targets and timeframes of change have not been defined. An international expert panel has been convened to develop 'treat-to-target' recommendations, based on published evidence and expert opinion. OBJ...

OBJECTIVES To compare the prevalence, clinical and radiographic characteristics of psoriatic spondyloarthritis (PsSpA) in psoriatic arthritis (PsA), with ankylosing spondylitis (AS). METHODS A prospective single-centre cross-sectional observational study recruited consecutive PsA and AS cases. Participants completed outcome measures, and underwent clinical examination, axial radiographic scor...
